What is TCA CROSS?

The use of trichloroacetic acid chemical reconstruction of skin scars (TCA CROSS) has been found to be a successful method for treating acne scars. This treatment targets ice-pick acne scars by utilizing high concentrations (55-75%) of TCA. Ice-pick acne scars can have a substantial impact on a person’s psychological well-being and social life. Fortunately, the TCA CROSS procedure is a relatively inexpensive, safe, and effective solution to address these concerns. The procedure involves the direct application of small amounts of TCA onto the surface of the ice-pick scar, which then results in a localized inflammatory reaction that promotes the formation of new collagen fibers. The goal of this treatment is to reduce scar depressions and increase collagen production to improve the appearance of the scar. However, it is essential to note that complete resolution of the scar is unlikely to be achieved through this treatment method alone. Most patients require 3-6 treatments of TCA CROSS for optimal improvement.

What to Expect Following TCA CROSS:

  • Frosting may last up to 12 hours
  • Skin surrounding treated areas may be red and sore for 24-48 hours
  • Scabs may develop after 2-3 days and will fall off on their own after 3-7 days
  • Sunscreen is required to reduce the chance of pigmentation
  • Schedule next session at 2 to 8-week intervals

Potential Complications of TCA CROSS:

  • Prolonged irritation and swelling
  • Damage to mucosal areas (i.e., lips, nostrils, eyes) causing ulcerations and potential scarring if TCA inadvertently applied on these areas
  • Post-inflammatory hyperpigmentation (usually temporary) or hypopigmentation (can be permanent)
  • Joining of surrounding scars to form larger scars
  • Suboptimal response or lack of improvement
